The name of the flag follows the same pattern as for regular flags: if ideology-specific, COSMETICTAG_ideology.tga, for the non-ideology fallback COSMETICTAG.tga. If a cosmetic tag doesn't change the appearance of a cosmetic aspect, it will remain unchanged from the default after the country gets it.
